Go to Website, then click 'Manage'

If you already have a contact page, click on it to edit. If not, you can create a new page for your contact form by clicking the 'Add another page' icon

Scroll down to the 'Content' section. Click the Available plugins dropdown menu and select the contact form that you need. We have 'contact details' and 'contact details with additional info' to choose from

Once you have chosen the contact form that you need, you can set it up to show that labels that you want shown or add additional labels that you need to capture the info that you need.

Make sure to click on the save buttons: one for the contact form plugin, and the save button for the contact page
